Non-invasive cardiac magnetic resonance and electrical myocardial imaging assessment of CRT in patients with heart failure and left bundle branch block

چکیده
Introduction CRT is an effective treatment for patients with heart failure and intraventricular conduction delay. However, clinical non-response rates remain unacceptably high; emphasizing the need for improved selection criteria. Central to the underlying mechanism of resynchronization is the relationship between mechanical and electrical activation. The purpose of this study was to demonstrate that mechanical and electrical activation can be quantified in patients with CRT devices combining cardiovascular magnetic resonance (CMR) and electrical myocardial imaging (EMI). Purpose Non-invasively characterize mechanical and electrical activation patterns in patients undergoing cardiac resynchronization therapy (CRT)

Purpose: To assess the relationship between the severity of coronary arteries involvement and the extent and pattern of myocardial scars in Cardiac Magnetic Resonance of patients with history of remote myocardial infarction. Materials and Methods: The Cardiac Magnetic Resonance images of sixty patients with history of remote ST segment or non-ST segment elevation myocardial infarction were ...
متن کاملCircumferential myocardial strain in cardiomyopathy with and without left bundle branch block
BACKGROUND Cardiac resynchronization therapy (CRT) has been shown to decrease mortality in 60-70% of advanced heart failure patients with left bundle branch block (LBBB) and QRS duration > 120 ms. There have been intense efforts to find reproducible non-invasive parameters to predict CRT response. We hypothesized that different left ventricular contraction patterns may exist in LBBB patients wi...
متن کاملCine DENSE MRI detects delayed mechanical activation of the left ventricular free wall in a canine model of heart failure with left bundle branch block
Background Cardiac resynchronization therapy (CRT) benefits selected heart failure patients; however, CRT has a nonresponse rate of approximately 30-40%. Important factors that contribute to lead placement decisions and overall CRT success include determination of mechanical dyssynchrony, presence and location of scar, and the identification of late-activated segments. Recent imaging studies ha...
متن کاملEvaluation of Cardiac Mitochondrial Function by a Nuclear Imaging Technique using Technetium-99m-MIBI Uptake Kinetics
Mitochondria play an important role in energy production for the cell. The proper function of a myocardial cell largely depends on the functional capacity of the mitochondria. Therefore it is necessary to establish a novel and reliable method for a non-invasive assessment of mitochondrial function and metabolism in humans. Although originally designed for evaluating myocardial perfusion, 99mTc...
متن کاملCardiac Resynchronization Therapy in Non-Ischemic Cardiomyopathy.
Cardiac resynchronization therapy (CRT) is an established therapy for heart failure patients who remain symptomatic despite optimal medical therapy, have reduced left ventricular ejection fraction (<35%) and wide QRS duration (>120 ms), preferably with left bundle branch block morphology. The response to CRT depends on the cardiac substrate: presence of correctable left ventricular mechanical d...
